Last Friday we told you the San Francisco Fire Department released its official report into the deaths of Lt. Vincent Perez and Firefighter/Paramedic Anthony Valerio. At the time, only a summary was available online. Now you can read the entire report:

My 2 cents from California.
The "Safety Investigation LODD Report" was revealing in some ways.
While San Francisco FD is large and in a Metropolitian Area of the State, some of its basic operations procedures need to be upated to confrom what the rest of us are doing.
While I was reading the report where it was discovered that the two firefighters, Perez and Valero were missing, no one called a "Mayday" let alone the IC calling for;
"Emergency Traffic" (on his radio) to gain control of Communictions, as outlined in; "Firescope Publications 420-1 Chapter 21/ICS 910 p-2."
Firefighter Emergencies
"The IC shall acknowledge the person in trouble and use the term "Emergency Traffic" to clear radio traffic".
This is just one example out of 25 that were mentioned for needing improvement.
It's not that the SFFD has not had previous experience with FF emergencies, there were two major ones prior to this one.
Hopefully as the leadership in the SFFD the changes (due to retirements) that are recommened are followed through on and a more pro-active approach is taken.
